BCHC Welcomes Main Street Health to Primary Care Clinics to assist Medicare Patients
Buchanan County Health Center (BCHC) recently partnered Main Street Health; the nation’s largest provider of value-based care focused exclusively on serving rural clinics. Based out of Independence, Jesup, and Oelwein primary care clinics, the Main Street Health team assists Medicare patients with care coordination, transportation, patient education, community resources, medication costs and financial assistance, and Medicare paperwork and benefits to make sure BCHC providers have accurate information at the point of care.
Medicare patients may receive calls from the health navigators on behalf of BCHC. These calls are to provide patients with free education and assistance for the best possible access to care.
