What happens if I need additional time before I’m able to care for myself at home?
Buchanan County Health Center Swing Bed Program is designed to meet your needs as you begin to recover from your illness or surgery. Often, additional time is necessary before you are able to care for yourself at home. This is when our Swing Bed Program can help you to recover to be independent at home. After an evaluation by our licensed professionals, our experienced group of physicians, nurses, therapists, dieticians, and social workers bring together a team responsible for your care. Personal goals are set by you and personalized by the Swing Bed Team. When those goals are met, you will be discharged from the program – either to your home or to another facility for longer rehabilitation. The Swing Bed Program is often covered by Medicare, Medicaid and many private insurances. For further details on the Swing Bed Program, please ask your health care team or call 319-332-0999.
